Survey Templates

Survey templates are ready-made surveys written by Give River. Pick one, adjust anything you like, choose who gets it and send it. It's the quickest way to run a good survey without writing every question yourself.

Starting a survey from a template

  1. Go to Engage > Surveys and click Create Survey.
  2. Choose Use a Template. The Choose a Template screen lists every template, each with a picture, a short description, how many questions it has and the drops it rewards. Templates that are always answered anonymously show an Anonymous label.
  3. Click the template you want.
  4. On the details step, the title, description and drop reward are filled in from the template. Change any of them, set the schedule and click Continue.
  5. The template's questions are now in your survey. Edit, remove, reorder or add questions as you like.
  6. Pick your participants, preview the survey and send or schedule it.

Your survey is your own copy. Changing it never changes the template, and later improvements to a template don't change surveys you've already made from it.

Templates you've already used

When your team has already made a survey from a template, its card shows an Added badge.

  • Click Open your copy on the card to go straight to the most recent survey you made from it.
  • Click anywhere else on the card to start another survey from the same template, for example when you want to run it again next quarter.

You may see more templates in the list as well. Anonymous templates start with Anonymous switched on. eNPS surveys are always anonymous. For the others you can switch it off on the details step before anyone answers, but people tend to answer more honestly when their names aren't attached. For anonymous surveys, results appear once at least 5 people have answered.

eNPS

Choosing the eNPS template opens the usual eNPS setup, where you pick how often it repeats (Monthly, Quarterly or Custom). Every occurrence then feeds the eNPS score on the Insights dashboard. See Surveys (Including eNPS).

Who's on your team?

Each of the 12 questions describes a moment at work and offers four answers. People pick the one that sounds most like them. Every answer stands for one of four team roles, but people only see the answers, not the roles, so nobody is steered toward the role they'd like to be seen as. The survey's description tells them their admin sees the mix of roles in the results.

  • Leader: sets the direction and makes the call when it counts.
  • Executor: turns plans into action and follows through on the details.
  • Cheerleader: keeps energy high, celebrates wins and makes sure people feel seen.
  • Quarterback: connects people and keeps the play moving, so the right person has the ball.

In the survey's results (Insights → Surveys & Polls, then the survey), a Team role mix card at the top shows how often each role was picked across all the questions, with a bar and a percentage for each. Under each question's chart, every answer is listed with its role next to it. A team that leans heavily one way can use this to talk about who naturally takes on which part of the work. If you made the survey anonymous, the role mix follows the same rule as the other results: it shows once at least 5 people have answered.

If you edit an answer or add your own, it shows without a role and doesn't count toward the mix.

Crucial conversation follow-up

Send this after a one-on-one or a 360 review conversation. People rate six statements from 1 (Strongly disagree) to 5 (Strongly agree), such as "The feedback was constructive" and "I felt heard", and can add notes. Answers are anonymous.
